STEPS TO REPRODUCE:
I was able to reproduce on Tahoe ToT ONLY under high CPU usage with the following

run-webkit-tests --no-retry --force —release --iterations 1000 http/tests/webcodecs/video-decoder-callbacks-do-not-leak.html

I am going to mark expectations as pass fail while this pends investigation.
