When images are disabled links in image maps are not accessible at all. We should present them as normal links alongside the image's alt text as a list or something. Spec: http://www.whatwg.org/specs/web-apps/current-work/multipage/section-the-canvas.html#image-maps (starting with "If the user agent intends")
