Issue happens in Safari 13 on both macOS and iOS.
Steps to reproduce for iOS:
1. Request camera stream via getUserMedia.
2. The red camera icon appears in Safari url bar.
3. Stop all media tracks.
4. Camera icon is hidden.
5. Request stream and stop it again.
6. Camera icon is present.
For macOS it acts similarly, but instead of Safari camera icon, the icon in macOS menu bar stays present.
On iOS it's quite bad, because if you collapse the safari application you will see a red bar on top telling you the website is using your camera, even though it is not as the stream is stopped.
