Summary: Remove the the captured variable hole from the stack, and make SymbolTable capable of telling you whether any given entry is a captured entry (i.e. activation index) or stack entry (i.e. VirtualRegister)

This implies fixing DFG::clobberize() to know that VirtualRegisters and captured variables aren't necessarily the same thing.
